Is Hard to say goodbye Death is probably the only thing in the world that is certain in life. As humans, we are born, we grow, we reproduce and lastly we die. It is the natural order and as natural as it may sound, human beings have different reactions to such an event. Throughout our lives, we influence or are influenced by people; these influences manifest themselves in the form of raising a child; being a teacher, babysitting etc. In all of these interactions, we create lasting relationships with those whom we come across when death happens to one of the involved parties; emptiness follows the surviving person.As a future nurse, and due to the nature of my occupation, I may be involved in a situation where I must deal with impending death. Emotions will begin to happen, on both ends. On my end I will be trying to help the patient, and the patient coming to terms with his or her destiny. In the following pages, I will discuss how my responses to the â€Å"Reflections on dying† may impact a therapeutic relationship with a mother of a three years old child who has six months to live. In such discussion, you will learn about my thoughts, feelings, beliefs, and values regarding to death and patient relationships.Thoughts; Usually when I first hear of a death My thoughts on death begin with sorrow for the loss of a person who is significant on someone’s live. After meeting a patient who is a mother, and realizing that she does not have much time I will be deeply saddened to what is going to happen to her. I may develop feelings for the child’s well being. My second thought would be that of empathy. As a nurse I must show empathy, compassion, and strength at the same time towards patients. Showing emotions would most likely worsen an already sad situation.This patient is a mother of a three years old child; she will not see her baby grow up. Therefore, I will have the utmost empathy. Also, I will show strength. This is necessary to persuade the patient to live fully the time she has left. If I am the person breaking the news to her, I will be frank and to the point without keeping any information from the patient; since I believe that sugar coating the fact will lead to mixed feelings on the patient. I will tell her to make every day, every hour, and every second count and to enjoy her child in order to leave a lasting legacy.Feelings; after a while, my initial reaction to death Therapeutic relations with terminally ill patients will also involve mixed feelings. There will be a war inside of me, my thoughts will tell me to be strong, but my feelings will want me to feel extremely sad, sorrowful, and incompetent for not being able to help the patient. These feelings will linger every attempt to interact with the patient. It is natural for me to feel sad at the future loss; even when I’ve only known the patient since she started getting treated; still all life is precious. She is meaningful to many people, specially her child.I will definitely feel impotent, which will lead to anger for knowing that there is nothing that I can do to make her feel better. At that point, I will rely on my professionalism to expose and help me act accordingly. The reason why my feelings are normally strong is because when I was twelve years old, my grandmother died; she practically raised me. I spent more time with my grandmother at that time, than I had spent with my mother. I was asleep when I was told that she had died, and feelings began to run rampant. The anger, and sadness were unbearable, and ever since that moment, I have been very sensitive when it comes to death.Beliefs; My predominant emotion to death I believe that all life is valuable; this belief will influence my feelings but not my therapeutic relationships with my patient. Regardless of what I may believe, I know that it is my duty to provide treatment the best possible service to the patient. Being supportive to the patient, and her family membe rs are my priority. If I was in the patient’s shoes, I would like the nurse to bring me up instead of bringing me down in a time of need. I would want the nurse to tell me my condition as is; that way, I can make the right decisions with my life.Also I would like to have everything clear when it comes to my child, and I would like to live my couple of months as happy as possible and pain free. Values; the way I may grieve Integrity is one of the main values I hold dear. A person’s word is their bond; without integrity, our words mean nothing. From integrity, respect and dignity follow on my list of values. My thoughts will influence ethical decisions when conducting therapeutic counseling. They will also reaffirm with my feelings to ensure I manage myself with strength. In addition, treating all with respect and dignity are a priority in my values list.Therefore, when dealing with a patient; everything else stops and I pay full attention to the patient’s needs. I will provide information and suggestions on ways to stay healthy and happy, even thought the patient won’t survive for long I feel that is best to keep the patient happy and comfortable. Conclusion In the last few pages, I’ve explained my thoughts on death. How my initial reactions to death would be and how my thoughts would influence me. Then, I explained how my feelings would manifest, and how they would play a part during routine interactions with the patient.I shared my beliefs and the connection between my beliefs, and thoughts when dealing with loss. Finally, I discussed my values as they relate to death. No matter which situation we find ourselves in; being the ones receiving bad news or giving the bad news; we will react differently to such an unexpected event. As long as we treat others with the respect and dignity they deserve, and we show the empathy we can make a difference on a patient’s remaining lifetime. It does take courage to show strength, a nd humility to show empathy, but the satisfaction to know that we have done the right thing would provide comfort to keep us going.

Does Science After All Rule Out A Personal God? Essay

Since time in memorial, science and religion which contains the idea of a personal god and his existence as one of its key pillar aspects, have always depicted an outstanding collide. Many scientific scholars and theologists have been involved in this argument in a bid to defend their different stands. However as long as this heated debate has existed, no answer has evidencial conclusion has been arrived at. For instance, the views contained by Albert eisten and Paul Tillich depict the difference in opinion about this aspect of a personal God. as much as everyone is entitled to his/her own opinion, these opinions and the justification they have do not seem to offer any direction to this long standing debate. Einstein states that there is no personal god. He gives a reason to justify his view stating that he and every other intellect is in a state that he similates to that of a little child getting into a huge study with books written in many different languages. He says this child knows that these books have their authors but he/she does not know and neither does she/he comprehend the books’ arrangement. He concludes that this is the perspective held towards god not only by him but by almost every other human having intelligence. After a comparison he did between the magnanimous relay and arrangement of the cosmos and failure to reconcile these features with the evil and suffering he got in the human’s being, eisteen concluded posing a question , how could an almighty god if at all he is there allow the existence of the suffering that is found on earth? In summing Einstein’s opinion he says that his idea is a childlike. However to him an embrace to the idea would be naive and anthropological . On the other hand theologist Paul Tillich holds a different opinion on the same. he asserts the existence of a personal god describing this being as the stronghold of being and even as the existence itself. Tilich, being a theologian is assertive and upholds the idea that a personal god actually exists despite the occurances that may question the existence of this being in terms of pain ,evil and suffering that exists among the human race. Tillich also states that God’s existence is not an aspect that can be proved or disapproved. In more bids to put more emphasis on his opinion which was commonly upheld by other theologists , Tillich even turned scientific tools into theological instruments. He was utterly committed to attempt put his view in the crystal clearest way that god is beyond essence and existence. Other scholars also involved in this argument like Hartshine say that there is really no sense in trying to find out whether a personal God exists. He however states that if there is existence of a personal God is a possibility then there is a necessity of doing so. In this bid it seems that there is a clear cut between those who agree with Tillich’s argument and those who differ with him. However these efforts are slowly being thwarted as the clock ticks. They would be plotting and killing Americans across the world and within our own borders. By fighting these terrorists in Iraq, Americans in uniform are defeating a direct threat to the American people†(Crimes Against Humanity). This is the government’s claim but it was never substantiated by any evidence, thus keeping the Americans wondering what really is the cause of this massive bloodshed. Some reports say that this war is in large part an oil currency war. â€Å"One of the core reasons for this upcoming war is this administrations goal of preventing further Organization of the Petroleum Exporting Countries (OPEC) momentum towards the euro as an oil transaction currency standard†(Mother Jones). In order to stop the OPEC from doing this, the US needed to take control of the OPEC’s 2nd largest oil producer, Iraq. This means that the Iraq War is just another economic war. It is just another result of the struggle to control the means of production. These taxes in addition to the economic pressures on the middle class—stagnant wages, the need to pull down two salaries to support a family, and the rising costs of the basic expenses—drive these families to turn to credit just to make ends meet. Credit in itself is neither bad nor is it taboo but the present state of the country prevents the middle class families from ever recovering (FromTheWilderness. com). The Iraq war brought about a change not merely on financial aspects but also in the way Americans perceive things. It has awakened the people’s social consciousness. Due to the doubtful claims the government are using to justify the war coupled with the suffering of the middle class, the masses are starting to wonder and ponder on the real issues that they are presently facing. A sense of awareness has donned on them, the people are slowly gearing towards social change. Having the freedom of intercourse, the people are slowly unearthing the reasons behind the war, the cause of their social helplessness and the means or solutions they are entitled to. Social justice is the concept of society in which justice is achieved in every aspect of society, rather than merely the administration of law. It is generally thought of as a society that affords individuals and groups fair treatment and a just share of the benefits of society. The Iraq war is not the sole cause of this change, several social forces are at work here but it may very well be considered as one of the factors that triggered it. After years of following the capitalist norm, the uneven distribution of wealth and power between the classes is already taking its toll. The delineation of the social classes is becoming clearer, each class is grouping itself together and yet, each one is dependent upon the other, without the bourgeoisies there would be no proletariat, without the proletariat there would be no cause for awareness, without a cause there can be no change. The social classes are now playing their important roles in this changes, they are setting the stage for their struggle. The United States is experiencing social change and it is inevitable. According to Neo-Marxism, strains are inherent in social structures and the source of these strains or contradictions is the inherent scarcity of certain goods and values. Thus inequality is a source of conflict. The conflicts caused by the inequality of the classes are starting to show. The ghost of communism is slowly rearing its head in our country. The Americans are preparing to experience a class struggle. The middle class is slowly becoming aware of the distribution of wealth and power between the masses and the selected few, they are realizing the exploitation of their class by a selected few using the government as an instrument. The country is now at the brink of a social revolution, just as how all revolutions came to be, the birth begins with the proletariat’s struggle with the bourgeoisie. At first the contest is carried on by individual labourers, then by the workpeople of a factory, then by the operatives of one trade, in one locality, against the individual bourgeois who directly exploits them and eventually the rise to power of the middle class. At this point however, the movement’s success cannot be ascertained. The thinking masses is too small to resemble a threat to imperialism but a war gone badly might alter public thinking enough to make such fundamental social change an increasing possibility. The ideas of Communism are based on actual relations springing from an existing class struggle, from a historical movement going on since time immemorial, it seeks to abolish property relations and to place power on to the working class (Mondo Politico). According to Karl Marx, this is the end result of all modern governments. Everything shall start from the bourgeois controlling first the modes of production hiring the proletariat only for labor, later on educating the proletariat so the bourgeois can use the knowledge endowed to the working class to remain competitive. Greed being the driving force, the bourgeois shall continue to teach and exploit the proletariat. Through this process, what the bourgeoisie, therefore, produces, above all, is its own grave-diggers. Its fall and the victory of the proletariat are equally inevitable. Communism is definitely an ideal type of government, it speaks of utopia where social justice reigns supreme and equity can never be flawed. The question now is that how come there is no country that has successfully established a perfect communist government? This might be caused by the degree of difficulty of implementing the transformation or it may be that eventually all systems shall need property relations of some kind. At this point, the social revolution in the United States is pretty far from fruition. Not all of the working class is willing to undergo change. After the United States has existed for so long, after having been accustomed to it and after achieving so much through capitalism, the premature abolition of property relations shall definitely cause its own downfall.

Reproductive Systems in Males and Females NAME : BENJAMIN NHANDARA Human Reproduction, Growth and Development All living organisms reproduce and it is one of their main  characteristics. Living organisms can produce a new member of species replacing those who might die due to old age, competition or in some cases being attacked by diseases. The process is known as reproduction (Clamp, A. 2005). Sexual reproduction involves two beings, male and female who have sex organs which produce gametes (sex cells) which then fuse together to form a new being. (M. B. V. Roberts, 2000). The journey begins in the male reproductive system. Hanging outside the pelvic cavity of a man lies a sack of skin which is known as a scrotum. Present in the scrotum are muscles which move the testicles slightly depending on the surrounding temperature. The scrotum contains the male reproductive organs testes that come in pairs. In the scrotum are muscles known as cremasteric muscles. Australia, H (2016) states that the scrotum helps to maintain the temperature of the sperm at slightly below body temperature (important for sperm  manufacture). The testes are the male primary sex organs which are oval and made of fibrous connective tissue. Producing male gametes (spermatozoa/sperm) is their main function. Each testis consists of approximately 300 lobules which are separated by connective tissue and filled with seminiferous tubules, were sperm cells are produced by meiosis as stated by McGuinness, H (2010). They are tightly coiled tubules. Interstitial cells also known as Leydig cells are found adjacent to the seminiferous tubules in the testicles. They promote the secretion of male sex hormone, testosterone. Production of male gametes begins at puberty (when a boy begins to develop into an adult, usually average age is 12 years in a male. Sperm contains the X and Y chromosomes (genetic information encoded with genes). The epididymis lies along the border of each testis and are long coiled tubes leading from the seminiferous tubules to the vas deferens. They store immature sperms until ready to be released by ejaculation (Centre, N.H.F. 2011). Long tubes known as vas deferens (sperm duct) are made of blood vessels and connect to the urethra. Sperm is released to the ejaculatory duct through the vas deferens. Short tubes join to the urethra from the seminal vesicles and semen passes through these tubes during ejaculation. The tubes are ejaculatory ducts and pass through the prostate gland. A muscular structure known as the urethra keeps urine in the bladder until excretion can occur. It provides a pathway for the flow of urine and secretion of semen. A sphincter muscle is there to prevent both functions occurring at the same time (McGuinness, H 2010). Penis is the main external male sex organ, surrounded by erectile and connective tissue. A foreskin (folded skin) covers the tip of the penis and has many sensory receptors. The penis hangs out the pelvic cavity and excretes urine a waste product. This main organ also penetrates and ejaculates semen inside the female reproductive organ during sexual intercourse (Chard, 2001). Males have reproductive accessory sex glands which are the Cowpers glands also known as bulbourethral gland, prostate gland and the seminal vesicle. Chard (2001), goes on to say that the Cowpers gland secretes a fluid that contributes to the seminal fluid, the secretion is less than that of the prostate gland. A pair of glands make the Cowper`s glands and they open into the urethra at the base of the penis. The prostate gland is like the size of a walnut and it is found in the pelvic cavity in front of the rectum and behind the symphysis pubis. A thin, milky fluid is secreted during ejaculation to enhance the mobility of sperms. Then again thick mucus-like fluid which forms 60% of semen is secreted by the seminal vesicle (M. B. V. Roberts, 2000). The fluid is alkaline and contains nutrients that are added to sperm cells. Located above the urinary bladder, highly coiled tubular structures are the seminal vesicles.    MALE REPRODUCTIVE SYSTEM Figure 1 (eMedicineHealth,2017) The female reproductive system is adapted so that an egg cell (gamete) can be produced and then fertilized by a sperm cell. The developing offspring is carried by the female being provided with nutrition until it is ready to be born. After birth nourishments are provided to the new infant by the female. This is all made possible by the way the female reproductive system is structured and functions. Ovaries are female reproductive parts that come in pairs one on either side of the uterus. They produce gametes (eggs) by a process known as ovulation. Hormones oestrogen and progesterone are also secreted by ovaries. Eggs are produced by a structure called the cortex and held in a vessel inside the medulla. The ovaries attach to the uterus by ligaments known as ovarian ligaments. About 2 million immature eggs are contained in the ovaries (Chard 2001). The fallopian tubes are muscular and lined with ciliated cells. They are about 5cm long, extend from the sides of the uterus. Other names like the oviduct or uterine tubes is used for the fallopian tube. At the end of each fallopian tube are finger-like projections known as fimbrae which receive the released eggs and transfer them to the uterus. Fertilisation also takes place in these tubes. In front of the rectum is a vast area known as the uterus. The uterus is a pear- shaped muscular chamber that has thick walls (McGuinness, H 2010). The cervix is the neck of the uterus and it opens into 2 upper corners. The uterus wall is composed of three layers of tissue, the perimetrium which is the outer layer which covers the top part of the uterus. The middle layer is known as the myometrium (middle layer of smooth muscle fibres). It is responsible for the contractions during period of labour. Finally, the third layer is the endometrium (soft with thick layer of compound tubular glands). Th is area is shed off each month during menstruation (Care of the mother and baby during labour, 1977). An 8-10cm muscular and elastic tube called the vagina connects the internal female reproductive organs with the external. It discharges fluids like menstrual fluid, urine and mucus. Male semen is received by the vagina and it also provides a passage way for childbirth. Outer wall stretches during sexual intercourse. The external genital organs of the female are collectively known as the vulva. They consist of the labia majora (thick folds of skin where pubic hair grows), labia minora (which are lip-like flesh at the entrance of the vagina), clitoris (contains erectile tissue and suspensory ligament that connect to the symphysis pubis), opening of the urethra and opening of the vagina. The main functions of the vulva are urination, menstruation, childbirth and sexual behaviour. THE FEMALE REPRODUCTIVE SYSTEM Figure 2 (Human Body Diagram, 2016) A female goes through a 28-day cycle known as the menstrual or the ovarian cycle. The female menstrual cycle is divided into 4 phases. The first phase is known as proliferative phase which occurs on day 2 of the cycle. The hypothalamus releases a hormone known as Gonadotropin- releasing hormone(GnRH). The GnRH hormone is responsible for the secretion of luteinising hormone (LH) and follicle-stimulating hormone (FSH) from the pituitary gland. The FSH secreted by the pituitary gland will stimulate the development of several ovarian follicles. A hormone called oestrogen is then secreted by the follicle on day 6 because of the chemical messenger FSH and the follicle develops into graafian follicle. Oestrogen inhibits the secretion of FSH by the pituitary gland on day 14. The layer lost in the previous menstruation is replaced by the proliferation of the endometrium. All repairs will be complete by day 14. The same hormone oestrogen will cause the secretion of LH hormone by the pituitary gland. The rapid increase in LH hormone will trigger the release of the mature egg from the ovary to the fallopian tube around day 14. This phase is referred to as the ovulation phase In the next stage, there is secretion of nutrients by the endometrium in preparation for the implantation and this stage is the secretory phase. An endocrine structure known as the corpus luteum secretes oestrogen and progesterone. The progesterone inhibits the production of FSH. No new follicles are stimulated, the endometrium is maintained by the high levels of progesterone. A rupture of the uterine capillaries will occur when a decrease in oestrogen and progesterone levels occur. Endometrium is lost from the body as a waste product through the cervix, together with some blood. This process is known as menstruation. Hormonal changes will prepare the uterus for pregnancy but in the event, that the egg is not fertilised then the lining of the uterus is lost through the vagina (menstrual period). Figure 3 clearly shows the 28-day cycle and the different periods that a female goes through during her menstrual cycle. The result is different if the egg gets fertilised. When the mature female egg in the fallopian tube fuse with a male gamete cell due to having had sexual intercourse. The fusion of the male and female gametes to form a zygote is known as fertilisation (Clamp, A. 2005). During sexual intercourse millions of sperms are deposited into the female reproductive tract. Sperms are smaller than the female gametes and because of their structure (having a head containing the haploid nucleus), a tail (flagellum) for mobility. The sperms swim towards the mature egg through the cervix during ovulation (Bennet, 1985). Only a few hundred sperms get close to the egg because many will die of the acidic environment and some will flow out of the vagina. Early in pregnancy the embryo secretes hCG (human chorionic gonadotrophins). This hormone enables the corpus luteum to continue to produce progesterone, thereby preparing the lining of the uterus for the developing embryo. By testing for HCG in urine, it is easy to diagnose for pregnancy very early. Another hormone is the human chorionic somatomammotropin (HCS) also referred to as human placental lactogen (hPL). The hormone is secreted from the placenta to modify the metabolic state of the mother during pregnancy (mother`s glucose usage and release of fatty acids). Gray, (2012) says that childbirth is divided into three stages. The woman`s age, size of uterus and the number of previous pregnancies will determine the duration of the stages. Dilation is the first stage, were the cervix dilates because the foetus would have begun its descent a few days or weeks earlier. Uterus muscle wall contract rhythmically and the head of the baby breaks the amnion and the fluid is released. Dilation of the cervix can last to 6 12 hours. As the contractions of the uterus become strong, the baby is then forced out through the cervix down the vagina. This is called the delivery stage and can be 1 -2 hours. Oxytocin hormone stimulates powerful contractions that help to open the cervix to push the baby and placenta out and limit bleeding at the site of the placenta. While the fight or flight hormone (adrenaline) can slow the labour for the fearful females. It can be produced in large quantities because of fear (Bennet, 1985). The umbilical code is tied and cut once the baby has begun to breathe or cry (can live independently without the placenta). After birth is the last stage which starts immediately after child birth and ends with the placenta expulsion. There is the production and ejection of milk from mammary glands (Institute4learning.com, 2017). The process is known as lactation. Prolactin hormone (mothering hormone) helps to prepare a woman for breastfeeding. Eventually the uterus will grow smaller because of the continued contraction. While nursing, 5 10% of women become pregnant again. To fully go through the human life, they are stages that need to be accomplished. They are about 12 different stages and we will look at infancy or the developing child in detail. During child infancy (0-3 years), a child will become more independent as they grow and the body matures. The baby learns how to walk, talk, feed themselves. A lot of the children do not get to walk in the first year as their bones will be weak to support the weight. By being encouraged the child will experiment and develop more confidence in the ability to co-ordinate themselves (Institute4learning.com, 2017). Motor movement is the title given to the development of co-ordination movement. As early as 3 months the baby will develop different sounds. Sounds are used to show pleasure, laugh or cry as a way communicating their needs. At this stage the child will like to play so much. The most important hormone that is present is the human growth hormone (hGH) which influences the growth of bone and muscle tissue.
